I just learned the importance of deal memos last September during the Crystal Wright Packaging Your Portfolio Workshop. I cannot stress enough how key it is to finalize how your credits will be listed BEFORE your shoot or production begins because it is hard to negotiate/change after the fact...
Then in some instances, it doesn't matter how much due diligence you impart, they may still get your information wrong as you can see from my Rip the Runway experience...
I guess me getting caught on camera is proof enough that I was there. :)
